The Arizona Diamondbacks Foundation prioritizes three areas of need in our state -homelessness and low-income housing; indigent health care and youth development. Within youth development, sports and particularly baseball and softball are of significant interest to the Foundation. Therefore, the Diamondbacks Foundation has introduced the "Play Ball Scholarship" Fund, a specific fund to provide support to Arizona youth baseball and softball leagues that have players requiring scholarship funding.
A joint venture between the Diamondbacks Foundation and Chase's Big Fly campaign created the "Play Ball Scholarship" Fund. Funds are awarded for up to $5,000. Organizations cannot receive more than one award per year from the "Play Ball Scholarship" Fund.
A panel of Diamondbacks Foundation Board of Directors, the Executive Director of the Diamondbacks Foundation and the Community Affairs Department will review grant applications. The number of organizations funded by the Diamondbacks Foundation's "Play Ball Scholarship" Fund will depend upon the quantity and quality of grant requests received, as well as the success of fundraising efforts by the Arizona Diamondbacks Foundation.
"Play Ball Scholarship" applications will be reviewed on a quarterly basis during the calendar year. Grants are awarded on a case-by-case basis to those leagues with a substantial need for scholarship funding. Applications should be completed and submitted after league registration is through as funding is awarded as a reimbursement.

INSTRUCTIONS
The Play Ball Scholarship Application consists of three parts: Cover Page, Narrative and Scholarship Worksheet and Delivery.
STEP 1: Cover Page
Please complete the online cover page form. Upon completion, a copy of your cover page will be emailed to the email address provided.
STEP 2: Narrative and Scholarship Worksheet
Complete the Narrative portion of the application in a separate, typed document. Include a Scholarship Worksheet detailing how the scholarships will be awarded. View example worksheet »
The Scholarship Worksheet should accompany this application and should not be sent in at a later time.
